Insurance document indicating student’s name and parent/guardian’s name
Legal guardianship documentation
Proof of Residence (only one of the three needed)*
Utility bill with parent/guardian’s name
Home telephone bill with parent/guardian’s name
Authentic mortgage statement/rental lease with parent/guardian’s name
* If you are unable to provide one of the three above, three other proofs of residence will be accepted (for example, one cellular phone bill indicating address, one bank statement indicating address or one W-2 form indicating address).
Proof of Student's Grade Level and School Assignment
Please provide a copy of the student’s most recent original report card.
